About Us: Meals on Wheels Orange County is the largest nonprofit provider of nutrition and supportive services for at-risk older adults in Central and North Orange County. Our mission is to combat hunger and loneliness among older adults through nutritious meals, friendly visits, and safety checks, and to support families through day services. We deliver nearly 2 million meals annually to 20,000 at-risk older adults.
Location: Anaheim, CA
Hours: Monday - Friday, 7:30 AM - 4:00 PM
Hourly Range: $27.00 - $32.00 per hour (Hourly rate will be determined based on factors such as geographic location, skills, education, and experience)
JOB SUMMARY: A qualified professional who will have responsibility for identifying and assessing potential participants for Adult Day Health Care admission suitability and act as an advocate for participant and family to help solve problems.
ESSENTIAL DUTIES AND RESPONSIBILITIES: - Informs potential referral sources as to process for referral and admission to ADHC program.
- Carries out participant home evaluation.
- Prepares an initial and continuing assessment of psychosocial needs of participant and participates in team development of plan of care.
- Provides referral for persons not appropriate for Adult Day Health Care Services.
- Prepares quarterly progress reports in participant's record, signed and dated.
- Documents, signs, and dates services provided on day performed.
- Serves as liaison with participant's family, home and community agencies.
- Provides for periodic reevaluation and revision of plan care. Reassessment of participant every six months.
- Provides discharge planning for all participants.
- Provides counseling to participants and families individually or in groups.
- Member of the Multidisciplinary Assessment Team.
- Meets and consults with LCSW on a regular basis.
- Performs other duties as needed.
EDUCATION, EXPERIENCE AND QUALIFICATIONS: - MSW degree from an accredited school of social work required.
- Must have current certification in CPR and first aid training.
- Must have a valid California driver's license and valid automobile insurance.
- Must pass physical examination and obtain tuberculosis clearance.
- Bilingual in English/Spanish desired.
- At least one year's experience working with elderly or disabled adults desired.
PHYSICAL JOB REQUIREMENTS: - Frequently remains in a stationary position and traverses locations.

- Occasionally distinguishes differences or similarities in intensity or quality of odors.
- Occasionally moves, transports, and positions objects weighing up to 75 pounds.
