🌟 Hospice Certified Nursing Assistant (CNA)

📍 Territory: Genesee County | 🕒 Full-Time | 🚗 Field-Based

🩺 What You’ll Do As a Hospice CNA

• Provide personal care and support based on the patient’s Plan of Care, under the direction of a Registered Nurse.
• Assist with daily living activities that bring dignity and comfort.
• Monitor patient condition and report changes to the care team.
• Offer compassionate emotional support to patients and families.
• Help create a safe, supportive, and respectful home environment.
• Collaborate closely with RN Case Managers to ensure quality, coordinated care.
• Advocate for patients and help prevent avoidable hospitalizations.

What You’ll Need

• High School Diploma or GED
• CNA certification
• At least 1 year of hospice or CNA experience
• Valid driver’s license, auto insurance, and reliable transportation
• Ability to travel within Genesee County (approx. 50%)
• Able to lift/move 50–100 lbs and meet physical demands of patient care
